Appointments outside of Franklin County must be scheduled manually.

Once this form is received, you will be contacted about scheduling the appointment.

For these appointments, there is a travel fee in addition to the behavior consultation fee.

$60 travel fee for the following counties

For all other counties, the travel fee will be calculated on an individual basis.
